New patients: If you prefer, you can download the appropriate forms below and complete them in the comfort of your home. Then, bring them with you to your first appointment, rather than filling out the paperwork in our office.
If you have completed the required paperwork ahead of time, please arrive five minutes early to your initial appointment. If you are unable to complete it ahead of time, please arrive at our office at least 15 minutes early.
MEDICARE PATIENTS: Due to Medicare requirements, there will be more paperwork involved. For this reason, please arrive at least 20 minutes early.
Required paperwork to complete prior to your first appointment: Registration and Health History. Remember to also bring your health insurance card and the physical therapy prescription from the physician, if you were given one. Our Notice of Privacy Practices is available for you to download and view, if you choose.
